What information does the Application obtain and how is it used?
User Provided Information
The Application obtains the information you provide once you click the “Add to Slack” button and install it in a Slack workspace. Once installed, the application will only collect answers from the Slack team members (“co-founders”) who actively choose to participate in the joint survey.
At the conclusion of the survey, co-founder answers are merged and scored. An interpretation is then provided to both co-founders describing actionable steps to improve the relationship and topics of discussion. Specific answers and scores are never shared with the other co-founder.
All identifying information, including the Slack ID for the co-founders is first anonymized before answers are persisted. Individual co-founders can choose to delete their survey along with all associated records at any time both during the survey and at the conclusion of the survey.
Automatically Collected Information
The application collects the team ID (the Slack Workspace ID) when the application is installed. This information is never stored alongside survey answers or other identifying information. It is only used to track which workspaces have installed the application in order to uninstall at a later time.
Does the Application collect precise real time location information of the device?
This Application does not collect precise information about the location of your mobile device.
Do third parties see and/or have access to information obtained by the Application?
We will never share the information we gather with outside third parties. The information is only collected for your personal use in order to provide an interpretation at the conclusion of the survey.
If we are required by law to comply with a subpoena or similar legal process, we will share the information requested to the best of our ability. However, the information we store is anonymized, which limits our ability to target specific records for named users.
What are my opt-out rights?
You can stop all collection of information by the Application by cancelling your active survey and uninstalling the Application. Say “cancel survey” or enter the slash command, “/eq cancel”, and you will be presented with multiple options to delete the active survey and/or all surveys you have completed.
Data Retention Policy, Managing Your Information
We will retain User Provided data for as long as you use the Application and for a reasonable time thereafter. If you’d like us to delete User Provided Data that you have provided via the Application, please say “cancel survey” or enter the slash command, “/eq cancel”, and you will be presented with multiple options to delete the active survey and/or all surveys you have ever completed. You may also contact us at firstname.lastname@example.org and we will respond in a reasonable time.
We do not use the Application to knowingly solicit data from or market to children under the age of 18. If a parent or guardian becomes aware that his or her child has provided us with information without their consent, he or she should contact us at email@example.com and we will delete such information from our files within a reasonable time.
We are concerned about safeguarding the confidentiality of your information. We provide physical, electronic, and procedural safeguards to protect information we process and maintain. We also anonymize all identifying information when saving results.
